Consent Form
The consent form describes common risks and benefits of psychotherapy, as well as my background and treatment approach, policies, expectations, and responsibilities as a therapist. It also describes client rights and responsibilities. Please read it carefully. I will be happy to answer any questions you have.

Intake Form
The intake form asks for a lot of information about your experiences and concerns. I ask clients to complete it and send it to me before our first therapy session, so that I don’t have to ask as many questions during our time together. It helps me understand more about you and the context of the concerns bringing you to therapy. It also helps me determine a diagnosis more efficiently — which is necessary if the session is being billed to your insurance company.
I do realize that some people are reluctant to share a lot of information about themselves before they’ve gotten to know someone. If you don’t want to answer some of the questions, you can skip them and just make a note about this on the last page.
